General Terms and Conditions By applying for the Concorde Community Healthcare grant, you agree to these terms: · You must be enrolled full time in a Concorde Career College/Institute program of study and in good academic and financial standing to be considered for a Concorde Community Healthcare grant award. · You may apply for and receive additional grants, grants or financial assistance; however, you may only submit an application for one Concorde Community Healthcare grant, and you only need to complete one form to do so. Entering additional profiles will not increase your chances of being selected. · You must provide your full name, email address, campus of enrollment, program of study, so that we can send you paperwork to complete if you are selected as a grant recipient. · Grants proceeds will be remitted to that school and applied as a tuition to the recipient’s academic balance. · By applying for the Community Healthcare grant, an applicant grants Concorde permission to post their name on www.Concorde.edu or in other marketing communications in the event they are awarded the grant. · Grant recipients may be required to submit a current photo and submit a publicity release granting Concorde permission to use their photo on www.Concorde.edu and in other marketing communications. · Concorde reserves the right to change the grant offerings at any time, including available grants, deadlines, requirements, award amounts or other program details. · Grant winners will be contacted by mail or in person within two weeks of the grant application deadline and may also be contacted by phone and e-mail. · If winners do not reply by the date specified in the award message, Concorde reserves the right not to award the grant, or to award the grant to another applicant or to redistribute the grant award dollars to another grant. · See eligibility requirements for each grant for more details
NO PURCHASE NECESSARY. A PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. CONCORDE COMMUNITY HEALTHCARE GRANT OFFICIAL RULES 1. ELIGIBILITY: The Concorde Community Healthcare Grant is open only to (i) enrolled Concorde College students in good academic and financial status who are residents of the fifty (50) United States and the District of Columbia, (ii) eighteen (18) years of age or older whose application for the grant does not violate any applicable law or regulation, including the Children’s Online Privacy Protection Act. and (iii) communicate in the form of a brief essay how they have helped improve access and/or quality of healthcare for minorities, (iv) submit a letter of reference from a 3rd party who can verify applicant’s service, and (v) provide proof of academic and financial standing may be required at any time. Employees of Concorde Career Colleges, Inc. (along with its parent entity, contractors, affiliates, and subsidiaries) and their families are not eligible. Void where prohibited by law. Online entries will be deemed submitted by the authorized account holder of the email address submitted at time of entry. 1. SPONSOR. The Concorde Community Healthcare Grant is sponsored by Concorde Career Colleges, Inc. (“Sponsor”). 2. HOW TO ENTER. Entrants may enter the grant by completing the online registration form on the page from which these Rules were linked (“Entry”). Any Entry that is submitted with an automated device or is not their original work is void. By submitting an Entry, entrant agrees to abide by these Official Rules and the Concorde Privacy Policy. Any Entry that does not comply with these Official Rules will be disqualified. 3. WINNER SELECTION. One (1) winner per Concorde campus will be selected by a committee based upon a rubric that places greater emphasis on the compelling nature of the applicant’s submitted essay, among all Entries received and processed (“Winner”). If a Winner is disqualified for any reason, an alternate Winner will be selected through additional review of Entries as many times as necessary. The Sponsor’s decision is final in all matters related to this Grant. The odds of winning will be determined by the number of entrants into the campus’ pool of applicants. 4. PRIZES. The Winner will receive a tuition reduction discount to their current Concorde tuition in the amount of One-thousand dollars (“Grant Prize”) as indicated on the Entry form, in the form of an applied credit to the Winner’s tuition balance.
ALL EXPENSES, FEDERAL, STATE AND LOCAL TAXES AND COSTS ASSOCIATED WITH ACCEPTANCE AND USE OF THE GRANT PRIZE NOT SET FORTH IN THESE RULES ARE THE OBLIGATION OF THE WINNER. The awarding of any Grant Prize is contingent upon full compliance with these Official Rules and subject to Sponsor’s verification of Applicant and Applicant’s submission. Applicants agree to be bound by Official Rules and agree that if any Winner is found to have violated the Official Rules or otherwise does not meet eligibility criteria, Grant Prize will be forfeited and an alternate Winner will be selected. Applicants understand that Sponsor is not liable for injuries, losses or damages of any kind arising from participation in this Grant Program and acceptance, possession and use of Grant Prize. Sponsor is not responsible for any typographical or other error in the printing of the offer, administration of the Grant Program or in the announcement of the Grant Prize. Winners may not transfer or assign any Grant Prize and any attempt to do so is void. Sponsor reserves the right to substitute a prize of equal or greater value, in its sole discretion.
