Eduvision, Inc. dba Arizona College of Nursing/Arizona College of Allied Health (“Arizona College”) has my permission to use my image, name, identifying information, testimonials, and speech. Unless you restrict the types of uses, this permission is unrestricted, and Arizona College may make any use, reuse, publication, or republication of the images and information.

Arizona College also may give some of its rights and allow others to use my image, name, and speech for any reason I allow on this form.

Arizona College does not have to pay me to use my image, name, or speech.

Arizona College will use my image, name, and speech only for the reasons I have approved. Arizona College does not have to show me how they will use my image, name, or speech in advance. I understand it might not look the way I thought it would.

I understand this permission to Arizona College is permanent unless I cancel it. If I do want to cancel, I must tell Arizona College. I understand that after canceling, Arizona College will stop using my image, name, and speech but Arizona College will not be able to get back things already made public. I also understand that once my image, name, or speech is made public, Arizona College cannot stop other people from using them.

I understand that I do not have to submit this form. If I do not submit this form, Arizona College will not take any action against me and I may participate in Arizona College programs on the same basis as before, whether or not I acknowledge it.

Release and Waivers: I release Arizona College and its employees, officers, directors, and contractors from any legal responsibility or liability for the use or disclosure of this information to the extent allowed by this Authorization. I waive the right to inspect or approve the finished product. I also waive any right to compensation for the use, dissemination, reproduction, distribution, and/or display of my testimonial, photos, videos, or audio allowed by this Authorization. By submitting this form, I agree that I have read and understood this Authorization.
